Waiver of Liability
I desire to work as a volunteer for the organization and engage in the activities related to the organization. I understand I will not be compensated for my work.
I voluntarily release and hold harmless the organization and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from my volunteer work with the organization.
I understand the organization does not assume any responsibility for, or obligation to provide any assistance to me for any reason.
I expressly and specifically assume the risk of injury or harm in any activity performed for the organization and release the organization from all liability for injury, illness, death, or property damage.
Media Release
I hereby grant permission to the rights of my image, likeness and sound of my voice as recorded on audio or video tape without consideration or compensation.
I understand that my image may be edited, copied, exhibited, displayed, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears.
Agreement to Waiver and Release
By accepting this waiver and release on behalf of myself, or as a parent, guardian or supervisor, I confirm that I have seen this waiver and release, have read it, and agree to its terms.
